Good news! In accordance with recent OMH policy updates, we are now able to provide "Rapid Readmission" to
recently discharged clients. What does that mean? (Glad you asked!)
Historically, discharge ended all services (it still does), and any appointment request post-discharge required a new Intake (CA). Now with Rapid Readmission,
a new Intake is not required for a client who was discharged within the past 12 months. So, a recently discharged client can immediately resume Achieve services with their previous therapist (provided that the therapist has available time), or with a new therapist.
In order to do so, two steps are required (and a third might be appropriate).
- When meeting with the client, you must use the "Rapid Readmission"
form as the first session note. This note should appear as the event in your Tasks, scheduled for you by the Scheduling Department. Otherwise, this form can be accessed
by clicking Service Entry -> Add New -> Outpatient -> Rapid Readmission.
We built all the required elements for rapid readmission into
this note. Please look through it carefully, and complete all fields. All
consents must be re-obtained, and can easily be done so inside of this note (during Telehealth this consent can be obtained verbally and recorded within the elements of this note). You will also record the Admission Decision inside this note (if the client
wishes to continue after this session, click Admit, and if not, click Non-Admit).
- Treatment Plan Review is required in first session.
- A Comprehensive Assessment Update is required, as usual, if there was any significant life / mental health event or situation (e.g., psychiatric hospitalization, suicide attempt, etc). This update would be done by the
clinician, as usual, and should not delay the rapid readmission session.
This process is relevant to many situations, including where a client you are working with needs to be referred to a higher level of care (e.g., a Partial Hospital Program). You would discharge the
client after confirming the appointment / client beginning the new program (depending on the circumstances), and with the Rapid Readmission process,
now not be concerned that such discharge would causing a delay or extra hurdle for the client should he or she wish to resume treatment in the future at Achieve after completing the new program.
